The incident occurred when a stacked Nelipak pallet, weighing 592 kg, toppled over, pinning Mr. Snowden against a wall. Eyewitnesses reported that coworkers made efforts to lift the fallen pallet, ultimately requiring an unloading process to free him. Unfortunately, by the time the pallet was removed, Mr.

Snowden had already succumbed to his injuries.

An inquiry conducted by the Health and Safety Executive (HSE) has revealed significant deficiencies in the company’s safety protocols. The investigation indicated that the firm failed to carry out adequate risk assessments that accounted for the load, height, weight, and stability of the pallets.

Notably, the company stored these three-legged pallets without appropriate racking systems, resulting in the hazardous practice of stacking them at ground level.

While the company has a documented Procedure for Pallet Handling Policy, the investigation uncovered that not all employees in the relevant area had received the necessary training. Disturbingly, there was no evidence that Mr. Snowden had been acquainted with these essential safety measures. This oversight highlights a more extensive issue within the company regarding employee safety education.

Legal repercussions and the importance of workplace safety

Following a thorough investigation, Systagenix Wound Management Manufacturing Limited, also known as Scapa Healthcare Limited, located at 997 Manchester Road, Ashton Under Lyne, is facing significant legal action. The company has pleaded guilty to breaching section 2(1) of the Health and Safety at Work etc. Act 1974, which emphasizes the employer’s responsibility to provide a safe working environment.

The court proceedings highlighted several critical failings that contributed to the incident. These findings underscore the urgent need for rigorous safety protocols within manufacturing settings.
