The incident: A day of routine labor

On February 16, Liam Haydon, a 22-year-old factory worker with nearly two years of experience, was performing his duties in the lamb boning hall.

This facility is crucial for preparing lamb cuts, requiring thorough cleaning, especially during night shifts. During his shift, he interacted with a piece of machinery known as the Speco solids-liquids separation device, which separates fats from liquids in the production process.

Testimony from the prosecution indicated that Mr. Haydon approached the machine and observed that its protective cage was not properly secured. Believing the machine to be inactive, he lifted the cage to begin cleaning. Initially, he used a shovel to clear debris and attempted to utilize a jet washer for stubborn pieces of meat. However, after these efforts proved ineffective, he leaned over and inserted his left arm into the machine to dislodge the blockage.

Incident details

A serious incident occurred when a machine unexpectedly activated after an obstruction was removed. The prosecution reported that the screw conveyor, which had been blocked, restarted without warning. Mr. Haydon’s urgent calls for help were heard by a nearby colleague, who quickly rushed to his side. Upon arrival, the colleague immediately assessed the situation and saw Mr. Haydon’s arm being pulled further into the machinery.

Attempts to stop the machine included pressing the isolator button, and emergency services were contacted without delay. Tragically, the only effective solution to free Mr. Haydon from the machine was to perform an amputation of his arm, a decision made under extreme circumstances.

Investigations and training concerns

Following the incident, police were present at the location; however, the primary investigation was led by the Health and Safety Executive. This inquiry examined several aspects, including the adequacy of Mr. Haydon’s training. It was revealed that he had undergone training involving 167 separate cleaning instruction cards, but the duration of the relevant training sessions was alarmingly brief, lasting only between three to ten minutes.

As the trial progresses, allegations against Kepak Group highlight the company’s failure to ensure the health, safety, and welfare of its employees, particularly regarding the operation and maintenance of the Speco solids-liquids separation machine. The prosecution contends that the company did not take reasonable precautions to mitigate the risks associated with the machinery, ultimately leading to the tragic incident involving Mr. Haydon.
