Change the bathroom door
A bathroom door which opens inwards takes up valuable space, as you cannot use the space behind it. Turn it around so that it opens outwards, or change it for a concertina type sliding door.
Space saver furniture
It is cheap and easy to put up a bathroom cabinet on the wall for all your toothpaste, medicines etc. There is also no reason why you should not have ordinary wall shelves in a bathroom.
You can find a small shelving unit which fits next to the bath; some of these come on wheels so that they can be moved around.
The ones made of plastic specifically for the bathroom are a good idea.

It is easy to find organiser units for bath and shower gel, cleaning stuff etc. These usually fit against the wall at the corner of the bath, and have several shelves. This gets the clutter off the side of the bath, and leaves you with more space.

As you can see, no matter how small your bathroom is, it can be designed so that all the necessary items will fit in. If you can, take a look at a bathroom on a canal boat; many of these incorporate a number of space saving ideas in a very small area.
Finally, before you do any of this, de-clutter your bathroom. Removing unwanted items can save a great deal of space and perhaps complete re-design is not necessary.
